Soreo Hospice & Palliative Care is dedicated to ensuring that Veterans with a life-limiting illness have access to high quality hospice care. Because we recognize that Veterans often have unique medical, social, spiritual and emotional needs, our care focuses on understanding the difficult circumstances faced by many Veterans near the end of life, such as, war related medical diseases, post-traumatic stress disorder, depression, unresolved issues associated with the stresses of military service, and financial and benefit concerns.

• Immediate access to care 24/7/365 with a one hour response time to assess any change in condition, alleviate pain and discomfort, and address Veteran or family/caregiver questions and concerns.
• Regular and as needed home visits by a Registered Nurse to assess Veteran well-being, address new or recurring symptoms, provide education about the disease process, refill medication, and to ensure understanding of medications regimes.
• Home Health Aid visits 2 times per week for personal care and assistance with ADLs.
• Guidance and support for coping with the emotional and spiritual issues related to having a life limiting illness, caring for someone with a life limiting illness, and loving someone with a life limiting illness.
• Emotional and Spiritual Care to optimize coping skills and promote healing for Veterans experiencing unresolved guilt, shame, and resentment.
• Patient/family education for ensuring early recognition of the emergence or recurrence of symptoms, managing symptoms at home, meeting care needs, understanding the disease process,
• Delivery of medications and DME to patient’s residence and any time condition change warrants.
• Assistance with applying for VA Aid and Attendant benefits, ALTCS, and VA Burial benefits.
• Assistance with identifying, accessing, and obtaining community services to address unmet social needs.
• Bereavement support for 13 months for family members, caregivers, and loved ones after the Veteran’s death; including face to face and phone grief support, check-in phone calls, educational mailings, support groups. referral to community resources, and assistance with death notifications, insurance claims, and related tasks.
